You can apply for an CPA individual license as an original applicant, through endorsement from another state, or by reinstating an expired license. You can also apply for a CPA firm permit as an original applicant, or by reinstating an expired permit.
We will only accept forms that are provided on this page. If the form does not print out on your printer as it appears on the screen, contact the BOA office to request that a form be sent to you.
To expedite the processing time for your application, please ensure that all supporting materials required to complete the application are emailed, faxed, and/or mailed to the BOA office. Delays will result if the application is not complete.

When you have completed your online application, you will receive a transaction confirmation receipt at the email address that you provided. The transaction confirmation receipt will specify additional requirements and the appropriate form(s) will be attached. In the event that you do not receive a transaction confirmation receipt, please contact the BOA office.
Based on your online CPA Individual License application type, please fax, email, or mail the appropriate additional form required (below) to the BOA office. For example, if you completed an online CPA Original Application, you must complete the Verification of Experience Form.
